On February 28, 1994, the Department of Toxic Substances Control (Department) inspected Lodi Chrome, 316 N. Main Street, Lodi, CA 95240, which is owned or operated by Respondent. 2 . The Department hereby determines that Respondent violated the Hazardous Waste Control Law (Health & Safety Code section 25100 et seq. ) , to wit: 2 . 1. On February 28, 1994, Respondent violated Health and Safety Code, Section 25201(a) in that Respondent treated a hazardous waste, waste plating sludge with heavy metals, without obtaining authorization from the Department, and the Department assesses a penalty of $300. 00. 2 . 2 . The Respondent violated additional requirements as specified on page 3 , Determination of Violations and the Department assesses a penalty of $400. 00. 3 . Payment of the total penalty of $700. 00 is due within 30 days from the effective date of the Order. Respondent' s check shall be made payable to the Department of Toxic Substances Control, and shall identify the Respondent and Docket Number, as shown in the heading of this case.
