On February 28, 1994, the Department of Toxic <br /> Substances Control (Department) inspected Lodi Chrome, 316 N. <br /> Main Street, Lodi, CA 95240, which is owned or operated by <br /> Respondent. <br /> 2 . The Department hereby determines that Respondent <br /> violated the Hazardous Waste Control Law (Health & Safety Code <br /> section 25100 et seq. ) , to wit: <br /> 2 . 1. On February 28, 1994, Respondent violated Health and <br /> Safety Code, Section 25201(a) in that Respondent treated a <br /> hazardous waste, waste plating sludge with heavy metals, without <br /> obtaining authorization from the Department, and the Department <br /> assesses a penalty of $300. 00. <br /> 2 . 2 . The Respondent violated additional requirements as <br /> specified on page 3 , Determination of Violations and the <br /> Department assesses a penalty of $400. 00. <br /> 3 . Payment of the total penalty of $700. 00 is due within <br /> 30 days from the effective date of the Order. Respondent' s check <br /> shall be made payable to the Department of Toxic Substances <br /> Control, and shall identify the Respondent and Docket Number, as <br /> shown in the heading of this case. Respondent shall deliver the <br /> penalty payment to: <br /> Department of Toxic Substances Control <br /> Accounting Office <br /> 400 P Street, 4th Floor <br /> P. 0. Box 806 <br /> Sacramento, California 95812-0806 <br /> A photocopy of the check shall be sent to: <br /> Mr. Donald A.
