This should include the company name, address, EPA ID #, <br /> Unit #, Tier of Authorization, and Date of closure. <br /> If you plan to continue to use/begin use of the Drum Rinsing/Crushing unit, then I would <br /> require that you resubmit the following attached forms at least 60 days before starting <br /> use of the unit.
