(v#106 cc393_Retraining, v#106 HAZ WASTE
Self Inspection Form )
https://smartandfinal-
my.sharepoint.com/:v:/p/nicole adame/EZxMrwiuQ9RGnsfrQ3CkarABpzliAhdw6cYbd4geJ5Gp
VQ?e=hN0a65
Violation #110 - Failed to keep signed copy of manifests from the designated
facility for three years.
This violation has been corrected.
All manifests have been updated and present in the green hazardous waste binder in
the store. Copies of the manifests are included with the response.
(v#110 08-29-19 020590300JJK, v#110 12-18-18 012497656FLE)
Violation #111 - Failed to comply with uniform hazardous waste manifest
exception requirements.
This violation has been corrected.
All manifests are all accounted for, therefore no need to generate a hazardous waste
manifest exception report. Copies of all manifests are included in the response.
Violation #404 - Failed to inspect hazardous waste storage areas at least weekly.
This violation has been corrected.
The company policy is to inspect the hazardous waste storage area daily. Darryl
Dinson, Environmental Compliance Manager, counseled the Store Manager, David
Crookston, regarding the importance accurately reviewing/completing the hazardous
waste Inspection checklist. We have also created an audit to be completed by the
District Loss Prevention Manager every time they complete a store visit to ensure
compliance of our hazardous waste program.
Violation #605 - Failed to completely label containers or portable tanks of
hazardous waste.
This violation has been corrected.
The container labels were corrected to include the generator name, and the physical
state. The store manager, David Crookston, was also counseled and retrained, by
Darryl Dinson, on the minimum requirements for hazardous waste labeling. I have
included pictures of corrected labels affixed to the bins. In order to ensure clearer and
more accurate labeling, going forward we will be implementing a new labeling process
involving our hazardous waste transporter. Each time the transporter picks up our
hazardous waste they will generate a label that includes the title "Hazardous Waste",
the generator name and address, accumulation start date (to be completed by the
store), composition, physical state, and the hazardous properties. I have included an
exemplar of the label for your review.
(v#605 AF Label, v#605 CA Label, v#605 CB Label, V#605 OX Label, v#605 TX Label,
v#605 UW Label, v#605 Label Exemplar)
