SAN J O A N N Environmental Health Department <br /> Small Quantity Hazardous Waste Generator Inspection Report <br /> Facility Name: Facility Address: Date: <br /> Smart& Final#593 215 E Lodi Ave Lodi October 29, 2019 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 605 CCR 66262.34(f) Failed to completely label containers or portable tanks of hazardous waste. <br /> OBSERVATION: Hazardous waste containers were observed on-site with incomplete labeling. The containers <br /> were: <br /> 1.An Aerosols, Flammables, and Combustibles(AF) Hazardous Waste tote,containing a bagged broken liquor <br /> bottle and two bags of absorbent used to clean up the spilled liquor, was observed missing the following <br /> information from its hazardous waste label:the name of the generator,the complete address of the generator(city <br /> and state were missing),the physical state(was not marked),the accumulation start date(was crossed out), and <br /> the contents marked was inaccurate(glass cleaners,whipped cream-aerosol cans,and spray deodorant were <br /> marked, but not found in the tote)(see photos IMG 9130-5).According to Sarah Bonilla, Store Manager,the date <br /> marked on the bags of waste, 8/26,was the date that the waste was placed into the hazardous waste tote. <br /> 2.A Corrosive Bases Hazardous Waste tote, containing an un-bagged unknown blue liquid,was observed missing <br /> the following information from its hazardous waste label:the name of the generator,the complete address of the <br /> generator(city and state were missing),the physical state(was not marked),the contents(was not marked), and <br /> the accumulation start date was lacking the year(IMG 9136-7). <br /> 3.An Oxidizers Hazardous Waste tote, containing a bagged container of Simply Value Bleach and used <br /> absorbent,was observed missing the following information from its hazardous waste label:the name of the <br /> generator, the complete address of the generator(city and state were missing), and an inaccurate and incomplete <br /> accumulation start date of 4/23(IMG 9140-1).According to Sarah,the bleach was placed in the hazardous waste <br /> tote on 10/25/19, indicated on the bag of bleach and marked as 10/25. <br /> REGULATION GUIDANCE: All hazardous waste containers shall be marked with the following information: the <br /> words"Hazardous Waste", name and address of generator, hazardous properties, physical state, composition <br /> (contents), accumulation start date <br /> CORRECTIVE ACTION: Immediately label the containers as required for hazardous waste. Provide a corrective <br /> action statement and supporting documentation to the San Joaquin County Environmental Health Department <br /> (EHD)within 30 days. <br /> This is a repeat violation, Class ll. <br /> Overall Inspection Comments: <br /> Notes: <br /> -When asked how the facility manages its waste fluorescent lamps generated in the facility, Sarah Bonilla, Store <br /> Manager, stated that she takes the waste fluorescent lamps that the facility personnel are able to replace, such <br /> as those in the refrigeration units and in the meat refrigerated displays, to Home Depot. Sarah stated that she <br /> also takes the facilities waste batteries to Home Depot. Sarah did not have a record showing when,where, and <br /> the quantity of the waste fluorescent lamps and waste batteries are taken to Home Depot. <br /> The San Joaquin County EHD recommends that the facility keep a written record or log documenting the name <br /> FA0000698 PR0539384 SC001 10/29/2019 <br /> EHD 22-01 Rev.9/20/2019 Page 9 of 10 Small Quantity Hazardous Waste Generator OIR <br /> 1868 E.
