The Facility Employee Training Certificate from May 2019 contained no address or phone number in the designated operator section. All site employees must be trained prior to taking responsibility as a facility employee if after October 1, 2018, or within 30 days of hire if prior to October 1, 2018. The Facility Employee Training Certificate shall be complete and contain accurate information for all employees trained. Update the Facility Employee Training Certificate to contain the complete information and submit a copy of the updated form to the EHD. This is a minor violation. 209 CCR 2716(a-e) Failure to conduct the designated UST operator visual inspection at least once every 30 days. The designated operator failed to check and note the correct test dates for the triennial secondary containment testing on the visual inspection reports. The date is incorrectly listed as 6/19/17. The routine triennial secondary containment testing was last conducted on 10/31/16. During the inspection,the designated operator shall check that all required testing and maintenance for the UST system have been completed and document the dates they were done. Ensure that designated operators performing inspections at this facility are including all of the required information on the reports. The designated operator failed to check and note all required items on the visual inspection reports.The incorrect and missing information included the following from the 7/29/19, 7/1/19, 6/3/19, 5/6/19,4/8/19, 3/18/19, 2/19/19, 1/21/19, 12/26/18, 11/27/18, 10/30/18, and 10/1/18 inspection reports: -No NA answers were documented in Section V. -The question regarding sump sensor locations was left blank, despite sensors being listed as inspected. -The question regarding UDC sensor locations was left blank. -Section VI (the owner/operator signature section)was left blank. -The question regarding sump sensor locations was answered, but no sump sensors were listed. -CERS ID was left blank. -Secondary containment testing information was left blank. -Six UDCs were listed as inspected, but the site only has two dispensers. During the visual inspection,the designated operator shall check that all required testing and maintenance for the UST system have been completed and shall check all required items on the report.
