A work order was found indicating testing was <br /> conducted on 9/7/18. These records shall be maintained on site for at least three years. Monitoring records <br /> include: (1) date and time of all monitoring or sampling; (2) monitoring equipment calibration and maintenance <br /> records; (3) results of any visual observations; (4) results of sample analysis performed a lab or in the field; (5) logs <br /> of all readings of gauges or other monitoring equipment, ground water elevations, or other test results; (6) results of <br /> inventory readings and reconciliations. Immediately locate and maintain the missing test results for the last three <br /> years on site. <br /> This is a repeat violation, Class ll. <br /> 202 CCR 2715(c)(3), 2715(c)(4) Failure to maintain the "Facility Employee Training Certificate" on-site (after October 1, <br /> 2018). <br /> The Facility Employee Training Certificate was not used for employees trained after October 1, 2018. All site <br /> employees must be trained prior to taking responsibility as a facility employee if after October 1, 2018, or within 30 <br /> days of hire if prior to October 1, 2018. The Facility Employee Training Certificate shall contain accurate information <br /> for all employees trained. Update the Facility Employee Training Certificate to contain the accurate information and <br /> submit a copy of the updated form to the EHD. <br /> This is a minor violation. <br /> 209 CCR 2716(a-e) Failure to conduct the designated UST operator visual inspection at least once every 30 days. <br /> The designated operator used outdated inspection forms for the visual inspections. During the visual inspection, the <br /> designated operator shall check that all required testing and maintenance for the UST system have been completed <br /> and shall check all required items on the report. Ensure that designated operators performing visual inspections at <br /> this facility are using the correct forms for all visual inspections. <br /> A designated operator inspection was performed on November 15, 2018 and the next inspection was performed on <br /> December 19, 2018, 34 days later. The designated operator visual inspections shall be conducted within 30 days or <br /> less of the previous inspection. Ensure that all designated operator inspections are performed within the required <br /> time frames. <br /> This is a repeat violation, Class ll. <br /> 308 CCR 2636(f) Failed to maintain a continuous monitoring system that activates an audible and visual alarm. <br /> The 87 STP sump sensor failed to come out of the audible and visual alarm when tested and is not currently <br /> functioning to ensure continuous monitoring of the tank system. All monitoring equipment shall be maintained to <br /> activate an audible and visual alarm or stop the flow of product at the dispenser when it detects a leak. Correct <br /> immediately by having a properly licensed, trained, and certified contractor replace the failed component with a <br /> functional component (LG 113-x listed, if applicable) and obtain a permit within one business day from the EHD or <br /> comply with the 'No Permit Required' conditions.
