The most recent financial responsibility <br /> documents onsite and in CERS are dated 4/9/18. Current financial responsibility documents are required to be <br /> submitted annually. Immediately log into the California Environmental Reporting System (CERS)at <br /> http://cers.calepa.ca.gov/, and upload the required documents. <br /> This is a repeat violation, Class II. <br /> 209 CCR 2716(a-e) Failure to conduct the designated UST operator visual inspection at least once every 30 days. <br /> A designated operator inspection was performed on October 24, 2018 and the next inspection was performed on <br /> November 27, 2018, 34 days later. The designated operator visual inspections shall be conducted within 30 days or <br /> less of the previous inspection. Ensure that all designated operator inspections are performed within the required <br /> time frames. <br /> The following was observed on the designated operator's monthly inspection reports: <br /> -Overfill prevention test date was incorrectly noted, correct test date is 8/31/18. <br /> -The"NA" in sections VII through XI were not explained. <br /> -Tank and line tightness test information was left blank. <br /> During the visual inspection,the designated operator shall check all required testing and maintenance for the UST <br /> system have been completed and shall check all required items on the report. Ensure that designed operators <br /> performing visual inspections at this facility are including all of the required information on the reports. <br /> This is a minor violation. <br /> 306 CCR 2630(d), 2641(a)Monitoring equipment is not installed or maintained to detect a leak at the earliest <br /> opportunity. <br /> The 208 sensor in the T2: 87 STP sump(tank closest to dispensers)was observed to not be at the lowest point, <br /> preventing the sensor from detecting a potential leak at the earliest opportunity. Monitoring equipment shall be <br /> maintained to be able to detect a leak at the earliest possible opportunity. While the inspector was on site,the <br /> sensor was relocated to the lowest point of the sump by the service technician. Ensure that all monitoring <br /> equipment is maintained to detect a leak at the earliest opportunity. <br /> This was corrected on site. <br /> This is a repeat violation, Class II. <br /> Overall Inspection Comments: <br /> The monitoring system certification was conducted on 5/22/2019 and was not fully witnessed by EHD. Looked <br /> inside all sumps and UDCs. Verified sensor locations and piping communication.
