The designated operator failed to document all the alarms from the attached alarm history on the designated operator inspection reports and failed to check that they were responded to appropriately. Numerous alarms occurred on the UST system prior to a tank top upgrade being conducted in 2018. During the inspection,the designated operator shall review the alarm history since the previous inspection, check that each alarm was documented and responded to appropriately, and attach a copy of the alarm history with documentation taken in response to any alarms to the report. Ensure that designated operators performing inspections at this facility are including all of the required information on the reports. The designated operator failed to visually inspect the sumps for leaks,water, or debris during inspections during 2018; numerous alarms occurred in the sumps during this time while the site was waiting for repairs and a tank top upgrade. The designated operator noted numerous repeat alarms from month to month, but did not document sump inspections. During the inspection,the designated operator shall visually inspect all spill containers and UDC sumps, and any tank top sump that had an alarm for which there is no record of a service visit. Ensure that designated operators performing inspections at this facility are inspecting all of the required areas and noting it on the on the reports. The designated operator failed to check and note all required items on the visual inspection reports.The incorrect and missing information included the following from the inspection reports: -Section V was left blank. -Section III was left blank. -The question regarding UDC sensor locations was left blank. -Section V is being completed by the designated operator with infomraiton regarding the issues found. This section is to be completed by the owner/operator with a description of corrective actions taken in response to the issues listed in Section III. -Alarm histories were not attached to the 2/25/19 and 3/26/19 inspection reports. -The alarm history attached to the 12/24/18 report did not contain S alarms. -The 10/29/18 report was written on an outdated inspection form. -The No and NA responses are not documented in Section III. -The designated operator is incorrectly marking that all issues from previous inspections were properly followed up to even when listing repeat issues. During the visual inspection,the designated operator shall check that all required testing and maintenance for the UST system have been completed and shall check all required items on the report. Ensure that designated operators performing visual inspections at this facility are including all of the required information on the reports. A designated operator inspection for the following months was performed more than 30 days: -Conducted 11/23/18, next conducted 12/24/18 (31 days) -Conducted 12/24/18, next conducted 1/25/19(32 days) -Conducted 1/25/19, next conducted 2/25/19(31 days) The designated operator visual inspections shall be conducted within 30 days or less of the previous inspection. Ensure that all designated operator inspections are performed within the required time frames.
