Environmental Health Department <br />Small Quantity Hazardous Waste Generator Inspection Report <br />Date: <br />May 03, 2019 <br /> Facility Address: <br /> 3538 Manthey Rd, Stockton <br /> Facility Name: <br /> Dollar Tree #05525 <br />SUMMARY OF VIOLATIONS <br />(CLASS I, CLASS II, or MINOR - Notice to Comply) <br />RemarksItem # <br /> 405 CCR 66262.34(d)(2) Failed to manage incompatible wastes in containers properly. <br />A tube of Coloreazy Color Permanent Cream Hair Color and a container of Coloreazy Developer, both for coloring hair, <br />were found in one bag inside an STOU HW tote. When Marlene scanned the barcode on the box in which both items <br />came in, the hand-held scanner indicated the entire package should be placed in a STOU HW tote. However, <br />ammonium hydroxide was listed as an ingredient in the cream hair color and hydrogen peroxide was listed as an <br />ingredient in the developer. According to the Dollar Tree Hazardous Material Management poster, items containing <br />ammonium hydroxide are classified as CB and items containing hydrogen peroxide are classified as OX. Marlene <br />separated the cream hair color from the developer, placed the cream hair color in the CB HW tote, and placed the <br />developer in the OX HW tote. (IMG 5851-57, 5862-3) <br />Incompatible wastes shall not be placed in the same container, or in an unwashed container that previously held an <br />incompatible waste or material.
