3. The Emergency Coordinator role was explained to the current ASM who assumed that <br />role. Her name and phone number were added to the Emergency Response/Contingency <br />Plan, and posted for access by all associates (EXHIBIT B). <br />4. Our hazardous waste vendor, Stericycle, submits manifests to the DTSC on behalf of the <br />company and has resubmitted the manifests. Please see the vendor letter of responsibility <br />(EXHIBIT C). <br />5. The manifests listed below have been provided to the store (EXHIBIT D). The manifests <br />were resubmitted to DTSC by Stericycle on April 26, 2019 with USPS tracking number <br />7018-3090-0002-1854-6903. <br />No. 011386009 <br />No. 011386010 <br />No. 011393308 <br />No. 011393307 <br />No. 010790445 <br />No. 010790446 <br />6. Three pails of paint found in the dumpster were determined to be hazardous waste and <br />were processed through the store’s hazardous waste program (EXHIBIT E). <br />7. Containers are labeled completely and correctly. Old labels were removed and proper <br />labels were created (EXHIBIT F). <br />8. The Assistant Store Manager has been trained to perform the weekly inspection <br />(EXHIBIT G). However, an EH&S Specialist will be conducting the Weekly Inspections <br />for the next 3 weeks. <br />9. Family Dollar store numbers had been converted when acquired by Dollar Tree. The <br />original store number was 11298. The current number is 31298 as shown in DTSC’s <br />system. However, the Family Dollar numbers in CERs are the old numbers. We have <br />begun the process of updating those numbers. <br />The signed certification is also included.
