They were all
containerized and placed in the appropriate Hazardous Waste Bin, to be picked up by our hauler and transported to
the TSDF. Store Staff and Management have been retrained to prevent reoccurrence. In addition, the store will be
monitored closely by our Regional managers, loss prevention, training staff, etc to ensure program in implemented
appropriately.

103: All of the items listed, with the exception of the shoe polish, were determined to be toxic, based on their
ingredient and placed in the designated Hazardous Waste Tote. The shoe polish was determined to be flammable
and placed in that tote.

104: Site map was updated to indicate spill control location. Also ensured emergency contact info is posted near the
telephone. (attached; site map and Emergency Contact Posting)

106: All Employees have completed Hazardous Waste handling training module for 2019 with the exception of two
who will be completing asap. All hazardous waste is being inspected by a member of management before it is placed
in its tote to ensure proper procedures are adhered to. (attached find training records for the store.) Training Module
content was sent to you as part of a previous inspection. Additionally, I am partnering with our regional managers and
trainers to spend some time at this store to ensure compliance.

109: A copy of the Manifest 011434364 FLE (8/29/18) has been re-sent to the DTSC. I have attached a letter from our
Hauler noting the Manifest was sent in to the DTSC as per our standard procedure. I was informed the DTSC seems
to be having some issues with backlogged manifests and misplacing manifests is not uncommon.

110-111: Signed copy from the destination facilities of the following manifests have been provided to the store for
filing on site. 011434365 FLE (8/29/18) (copy attached).

202: Party String item was removed from the trashed and placed in the HW aerosol tote for proper disposal.

Please contact me directly if you have any further questions

Edgar Flores
Manager, Compliance

Cell 323-919-3290
Office 323-881-9950 x 1383
Fax 323-729-3455
4000 Union Pacific Avenue
City of Commerce, CA 90023
edgar.flores@99only.com
