Environmental Health Department <br />Small Quantity Hazardous Waste Generator Inspection Report <br />Date: <br />May 10, 2019 <br /> Facility Address: <br /> 450 S Cherokee Ln, Lodi <br /> Facility Name: <br /> Dollar Tree #06120 <br />SUMMARY OF VIOLATIONS <br />(CLASS I, CLASS II, or MINOR - Notice to Comply) <br />RemarksItem # <br /> 605 CCR 66262.34(f) Failed to completely label containers or portable tanks of hazardous waste. <br />- An Aerosols, Flammables, & Combustibles (AF) HW tote was found lacking the physical state on the HW label, <br />located in the HW storage area (IMG 6066). Andres marked the physical state on the AF HW tote at the time of <br />inspection. <br />- The items listed below were found in the mop sink located in the facility’s back storage area (IMG 6100-3). Andres <br />did not know how long the items had been in the mop sink. <br /> o Two empty 2 fl. oz. GS Neutral Cleaner Concentrate ST-846 packs. Andres stated he places the empty cleaner <br />concentrate packs into a HW bag and in the HW storage area, but does not place them into a HW tote because he <br />did not know which HW tote the cleaner packs belonged. Andres stated the cleaner packs are not sold in the store, <br />but purchased by the facility to clean the facility floors. Andres called a store manager of a Dollar Tree store in Galt, <br />CA to ask which HW tote the cleaner packs should be placed into. Andres stated that the store manager in Galt said <br />the empty cleaner packs are to be placed in a CB HW tote. Andres placed the two empty GS Neutral Cleaner <br />Concentrate ST-846 packs into a CB HW tote. <br />- Two empty 2 fl. oz. GS Neutral Cleaner Concentrate ST-846 packs were found in a black bucket located on a stack <br />of boxes next to the mop sink. Andres did not know how long the empty cleaner packs had been in the bucket. <br />Andres placed the two empty GS Neutral Cleaner Concentrate ST-846 packs into a CB HW tote. (IMG 6107-9) <br />- A HW bag marked “50 cents” and containing a leaky 25 oz. container of Comet With Bleach cleaning powder was <br />found for sale in a Dollar Tree basket labeled with a “Reduced Merchandise” pink tag, located at a check-out register. <br />Andres scanned the Comet With Bleach, and the hand-held scanner indicated the Comet should be placed in an <br />STOU HW tote. However, according to the Products To Be Treated As Hazardous Waste guide, “Ajax or Comet Type <br />Powdered Cleaner (with bleach)” is to be placed in an OX HW tote. Andres removed the HW bag containing the Comet <br />from the basket and took the item to the facility’s back storage area. Andres stated he would place the Comet into an <br />OX HW tote.
