Mr. Jack Verderame U - 5 - 10 March 2010 <br /> Requirements Section No. Applicable <br /> Subsurface Disposal Systems B.5.a,b When discharging to leachfield <br /> Surface Disposal Systems B.6.a,b,c,d When discharging to pond <br /> The Discharger submitted the Sludge Disposal Plan on 27 January 2010. <br /> The monthly average flow rate to the VWVTF is hereby limited to 20,000 gpd. (The <br /> monthly average shall be calculated by dividing the total flow for the month by the number of <br /> days\tt onth). In addition, the Discharger shall construct and begin use of a minimum of <br /> 2,200 linear et of Ieachfield by 2 August 2010. By 31 August 2010, the Discharger shall <br /> submit a Notice of Completion Report that describes the Ieachfield construction. <br /> Please review this Notice of Applicability carefully to ensure that it completely and accurately <br /> reflects the proposed discharge. If the discharge violates the terms or conditions above, the <br /> Central Valley Water Board may take enforcement action, including assessment of <br /> administrative civil liability. If the method of waste disposal changes from that described in the <br /> RWD and additional information, you must submit a new RWD_ <br /> Canada Cove LLP will generate the waste subject to the terms and conditions-of WOO <br /> No. 97-10-DWQ and will maintain exclusive control over the discharge. As such, Canada <br /> Cove LLP is primarily responsible for compliance with WOO No. 97-10-DWQ, this NOA, and <br /> the attached MRP. Failure to comply with the requirements in the documents could result in <br /> an enforcement action as authorized by provisions of the California Water Code. Discharge of <br /> wastes other than those described in the RWD is prohibited. <br /> The required annual fee specified in the annual billing from the State Water Board shall be <br /> paid until this NOA is officially terminated. You must notify this office in writing if the discharge <br /> regulated by this Order ceases so that we may terminate coverage and avoid unnecessary <br /> billing. <br /> WDRs Order No. 135-2006-0039 (NPDES Permit No. CA0083682) contains monitoring <br /> requirements. Those monitoring requirements are hereby suspended when discharge to the <br /> golf course pond begins. The NPDES permit will be rescinded at the May 26/27/28 meeting of <br /> the Central Valley Water Board. <br /> All monitoring reports, submittals, discharge notifications, and questions regarding compliance <br /> and enforcement should be directed to Brendan Kenny at (916) 464-4635 or <br /> bkenny@waterbaards.ca.gov.
