/ ABF Freight 2 <br /> 2747 Yosemite Avenue <br /> Manteca,CA <br /> WELL ABANDONMENT ACTIVITIES <br /> On July 8, 1999, CAL INC contracted with Bay Area Explorations, Inc (BREI) to abandon monitor wells <br /> MW-1, MW-2 and MW-3 A permit to abandon the three wells was issued by the EDH A copy of the <br /> well abandonment permit and State of California Well Completion (abandonment)Reports are to Appendix <br /> B The copies of the original Well Completion forms were forwarded to the Department of Water <br /> Resources in Sacramento, California and EHD to Stockton,Caltforrtta <br /> The three monitor wells were abandoned in place by pressure grouting Portland cement throughout each of <br /> the well casings and surrounding sand packs Portland cement was pumped into each of the wells using a <br /> PVC tremmte pipe and rubber pressure hose attached to the grout pump on BAEI's drill rig(see Appendix <br /> I C - Photograph #2) Once the cement had reached the top of each well casing, a PVC coupling was <br /> attached to the top of the well heads and additional Portland cement was pumped under pressure into each <br /> of the wells Each well took approximately 17 gallons of cement before the well casings and void space to <br /> the surrounding sand pack and formation had been filled <br /> Once the well casings were filled with Portland cement, the ground surface vault boxes covering the <br /> monitor wells were removed(see Photograph 4)and the resultant holes filled with concrete <br /> If you have any questions regarding this report,please give me a call at(707)446-7996 <br /> Sincerely, <br /> I �E13 Gfi? <br /> CAL INC�Gq-- 6031 <br /> JOMNVtrs� <br /> LODE <br /> 5 Q <br /> 9aFCA1.�F <br /> John Love,R G #6315 I=xp�•�s it-�°-ov <br /> Project Geologist <br /> I <br /> I <br /> I <br /> N 1PR0JECTS146381Rcports\C1osure Report doc <br />
