Environmental Health Department <br />AMENDED <br />11/7/2019 <br />Large Quantity Hazardous Waste Generator Inspection Report <br />Date: <br />October 22, 2019 <br /> Facility Address: <br /> 185 S UNION RD, MANTECA <br /> Facility Name: <br /> MEINEKE CAR CARE CENTER <br />SUMMARY OF VIOLATIONS <br />(CLASS I, CLASS II, or MINOR - Notice to Comply) <br />RemarksItem # <br />(3) a written description, including a syllabus and/or outline, of the type and amount of both introductory and <br />continuing training that will be given to each person filling a position listed under paragraph (d)(1) of this section; <br />(4) employee-signed or -certified records that document that the training required under subsections (a), (b), and (c) <br />of this section has been given to, and completed by, each employee. <br />(e) The owner or operator shall maintain training records on current personnel until closure of the facility and training <br />records on former employees for at least three years from the date the employee last worked at the facility. <br />Personnel training records may accompany personnel transferred within the same company. <br />CORRECTIVE ACTION: Immediately train all employees who work with hazardous waste in compliance with the <br />time frames and training materials required by this section. Provide training documentation to the San Joaquin <br />County Environmental Health Department (EHD) within 30 days. The documentation must include a list of training <br />topics, date of training, job title, name of employee, written job description (qualifications and duties), and employee <br />signed (or certified) record that documents the training. <br />This is a Class II violation. <br /> 119 HSC 25160.2(b)(4) Consolidated manifest receipts failed to contain complete information. <br />OBSERVATION: Consolidated manifest receipts found on-site for disposal of hazardous waste contained <br />incomplete information. According to Chris Dalen, owner, metal fines are added to the 55 gallon drum of oily <br />solids/paper filters for disposal. However, none of the consolidated manifests for oily debris from the past three <br />years included metal fines in the contents. <br />REGULATION GUIDANCE: (4) All copies of each receipt shall contain all of the following information: <br />(A) The name, address, identification number, contact person, and telephone number of the generator, and the <br />signature of the generator or the generator’s representative. <br />(B) The date of the shipment. <br />(C) The manifest number. <br />(D) The volume or quantity of each waste stream received, its California and RCRA waste codes, the wastestream <br />type listed in subdivision (c), and its proper shipping description, including the hazardous class and United <br />Nations/North America (UN/NA) identification number, if applicable. <br />(E) The name, address, and identification number of the authorized facility to which the hazardous waste will be <br />transported. <br />(F) The transporter’s name, address, and identification number. <br />(G) The driver’s signature. <br />(H) A statement, signed by the generator, certifying that the generator has established a program to reduce the <br />volume or quantity and toxicity of the hazardous waste to the degree, as determined by the generator, to be <br />economically practicable. <br />CORRECTIVE ACTION: Ensure the accurate California and RCRA waste codes, wastestream type, and proper <br />shipping description for all waste streams are included on the consolidated manifests.
