Environmental Health Department AMENDED 11/7/2019 Large Quantity Hazardous Waste Generator Inspection Report Date: October 22, 2019 Facility Address: 185 S UNION RD, MANTECA Facility Name: MEINEKE CAR CARE CENTER SUMMARY OF VIOLATIONS (CLASS I, CLASS II, or MINOR - Notice to Comply) RemarksItem # (3) a written description, including a syllabus and/or outline, of the type and amount of both introductory and continuing training that will be given to each person filling a position listed under paragraph (d)(1) of this section; (4) employee-signed or -certified records that document that the training required under subsections (a), (b), and (c) of this section has been given to, and completed by, each employee. (e) The owner or operator shall maintain training records on current personnel until closure of the facility and training records on former employees for at least three years from the date the employee last worked at the facility. Personnel training records may accompany personnel transferred within the same company. CORRECTIVE ACTION: Immediately train all employees who work with hazardous waste in compliance with the time frames and training materials required by this section. Provide training documentation to the San Joaquin County Environmental Health Department (EHD) within 30 days. The documentation must include a list of training topics, date of training, job title, name of employee, written job description (qualifications and duties), and employee signed (or certified) record that documents the training. This is a Class II violation. 119 HSC 25160.2(b)(4) Consolidated manifest receipts failed to contain complete information. OBSERVATION: Consolidated manifest receipts found on-site for disposal of hazardous waste contained incomplete information. According to Chris Dalen, owner, metal fines are added to the 55 gallon drum of oily solids/paper filters for disposal. However, none of the consolidated manifests for oily debris from the past three years included metal fines in the contents. REGULATION GUIDANCE: (4) All copies of each receipt shall contain all of the following information: (A) The name, address, identification number, contact person, and telephone number of the generator, and the signature of the generator or the generator’s representative. (B) The date of the shipment. (C) The manifest number. (D) The volume or quantity of each waste stream received, its California and RCRA waste codes, the wastestream type listed in subdivision (c), and its proper shipping description, including the hazardous class and United Nations/North America (UN/NA) identification number, if applicable. (E) The name, address, and identification number of the authorized facility to which the hazardous waste will be transported. (F) The transporter’s name, address, and identification number. (G) The driver’s signature. (H) A statement, signed by the generator, certifying that the generator has established a program to reduce the volume or quantity and toxicity of the hazardous waste to the degree, as determined by the generator, to be economically practicable. CORRECTIVE ACTION: Ensure the accurate California and RCRA waste codes, wastestream type, and proper shipping description for all waste streams are included on the consolidated manifests. Provide a corrective action statement and supporting documentation to the San Joaquin County Environmental Health Department within 30 days. This is a minor violation.
