Environmental Health Department <br />AMENDED <br />10/18/2019 <br />Small Quantity Hazardous Waste Generator Inspection Report <br />Date: <br />October 11, 2019 <br /> Facility Address: <br /> 8011 DON AVE, STOCKTON <br /> Facility Name: <br /> STOCKTON SURGERY CENTER <br />SUMMARY OF VIOLATIONS <br />(CLASS I, CLASS II, or MINOR - Notice to Comply) <br />RemarksItem # <br /> 104 CCR 66262.34(d)(2) No modified contingency plan. <br />OBSERVATION: A complete modified contingency plan was not posted at the facility. <br />REGULATION GUIDANCE: The business operator must post the following information next to telephones or in <br />areas directly involved in the generation and accumulation of hazardous waste: the name and emergency <br />telephone number of the emergency coordinator, the location of fire extinguishers and spill control material, and the <br />telephone number of the fire department unless the facility has a direct alarm. <br />CORRECTIVE ACTION: Immediately post the required information at the facility. Provide a statement to the San <br />Joaquin County Environmental Health Department that the correction is complete. <br />This is a minor violation. <br /> 105 CCR 66262.34(d)(2) No emergency coordinator. <br />OBSERVATION: A complete modified contingency plan with an assigned emergency coordinator was not posted at <br />the facility. <br />REGULATION GUIDANCE: The emergency coordinator or his designee must respond to any emergencies that <br />arise. The EHD has written the <br />complete report on October 18, 2019 which replaces the initial checklist. <br />Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br />documenting the corrective actions that have been or will be taken for each violation, and any supporting <br />paperwork, within 30 days of receiving the complete inspection report. <br />Waste Streams: waste ethyl chloride spray bottles, waste anesthetic bottles, IV tubing waste <br />Documents Provided: hazardous waste labeling information, contingency plan template, modified contingency <br />plan template, free CUPA classes flyer, Return to Compliance Certification and Corrective Statements report <br />Note: Sutter Health can combine the facilities at 2505 W. Hammer Lane, 2545 W. Hammer Lane, and 8011 Don <br />Avenue under one hazardous waste small generator program and one hazardous material business plan <br />program.
