Incident Details <br />3/11/19-present <br />Special Conditions: UVR 2040 has been in service from the startup date of 3/11/19. There is power monitoring <br />equipment in the main MCC measuring power to UVR 2040. UVR 2040 L-2-2 had low irradiance issues while <br />Calgon was on site on 3-14-19. An inspection was performed and found the well contaminated. L 2-2 well was <br />replaced and verified with a calibration. L-2-2 had 306 hours until lamp broke. <br />3/20/19 <br />1251: GFI fault for UVR 2040 alarmed. Heard water gushing from the UVR room indicating a lamp break. <br />1252: Matt Cordua donned PPE to close the MOV/FCV locally and to take initial Mercury readings. Initial <br />readings were 0 mg/m³. <br />1255: MOV/FCV were forced closed via MOP panel. <br />1300: MOV/FCV fully shut. <br />1305: All onsite personnel were notified not to enter UVR room. All Access denied to UVR room. <br />1310: Opened all roll up doors and access doors to ventilate area <br />1315: TTF Onsite Response team donned PPE to inspect UVR and number of lamps damaged. <br />1316: All proper personnel were notified via phone/email. <br />1330: Response team found that one lamp (2-2) broke. There were no pickups on the Jerome meter leading to <br />and around the Reactor however a reading of 0.27 mg/m³ inside the Reactor. <br />1345: Onsite samples were collected and placed in lab fridge waiting WQ pickup. <br />1400: Reactor LOTO’d and isolated from the system. <br /> <br /> <br /> <br />
