Time In: 9:30 am <br /> r <br /> Time Out: 9:58 am <br /> Food Program Complaint Inspection Report <br /> Name of Facility: MOKELUMNE BREW HOUSE Date: 08/12/2020 <br /> Address: 115 S SCHOOL ST, LODI 95240 <br /> Owner/Operator: FULTON, JAMES& EMILY/CLARK, KEVIN &KATHRYNNE Telephone: (209)981-3344 <br /> Program Element: 1600-FOOD PROGRAM Complaint#: C00052425 <br /> DESCRIPTION OF COMPLAINT ALLEGATIONS <br /> The San Joaquin County Environmental Health Department received the following complaint: <br /> COMPLAINANT ALLEGES THIS FACILITY IS NOT FOLLOWING SOCIAL DISTANCING PROTOCOLS,THEY HAVE LIVE <br /> MUSIC ON FRIDAY EVENINGS AND ARE STILL OFFERING DINE IN.THEY ARE ALSO NOT REQUIRING MASKS. <br /> COMPLAINANT ALLEGES THIS FACILITY DOES NOT HAVE A LIQUOR LICENSE BUT OFFER KAHLUA AND <br /> COCKTAILS AS AN OPTION.THEY DO NOT CLEAN TABLES AFTER EACH USE.A PATRON OF THIS FACILITY <br /> INFORMED THE FACILITY THAT THEY TESTED POSITIVE FOR COVID AFTER ATTENDING ONE OF THEIR MUSIC <br /> NIGHTS AND THEY DID NOT DO ANYTHING.THEY ADVERTISE THAT THE EMPLOYEES RECEIVE TEMPERATURE <br /> CHECKS BUT THE EMPLOYEES ADMINISTER THEIR OWN TEMPERATURE CHECK AND ARE NOT OBLIGATED TO <br /> REPORT ANYTHING. <br /> VIOLATIONS AND CORRECTIVE ACTIONS <br /> Items listed on this report as violations do not meet the requirements set forth in the California Health and Safety Code commencing with section 7; <br /> 113700.All violations must be corrected within specified timeframe. Violations that are classified as"MAJOR"pose an immediate threat to public health <br /> and have the potential to cause foodborne illness.All major violations must be corrected immediately.Non-compliance may warrant immediate closure of <br /> the food facility. <br /> OVERALL INSPECTION NOTES AND COMMENTS <br /> FOOD ITEM--LOCATION--TEMP°F--COMMENTS <br /> No Temperature Data Collected <br /> NOTES <br /> I went to the facility and spoke to the facility operator on the phone. <br /> I asked her if she was aware of the complaint. <br /> The facility operator said she remembers a phone call that her employee answered in regards to something similar in the <br /> complaint. <br /> I let her know that masks are required and that live music is not allowed at this time. <br /> I also informed her that there is no indoor dining.
