Process Hazard Analysis Report July 2020 <br /> • The list of 11 recommendations includes safeguards which may help to prevent <br /> or minimize the chances of an accidental release of ammonia occurring at Oak <br /> Ridge Winery. <br /> The PHA and revalidation entailed a review and update of the prior PHA which <br /> covered all three (3) ammonia refrigeration systems. <br /> As part of the review and revalidation, the team reviewed the prior human factor <br /> checklist. Any recommendations stemming from the review were noted in the <br /> appropriate column. The external events checklist from the prior PHA was <br /> reviewed and revalidated as well. The PHA team had the same conclusions as the <br /> prior team, that most events have little likelihood of occurrence or a potential to <br /> create a release of ammonia. Whatever the likelihood for human factor and/or <br /> external events, the team ensured the PHA was consistent with the current <br /> process due to modifications that had occurred. <br /> Oak Ridge Winery should document the corrective actions taken to address each <br /> team recommendation. The corrective actions which may include supporting <br /> documentation, like training documentation, pictures, etc. should be included. <br /> This PHA and revalidation report shall be kept for the life of the process, as <br /> required by the CalARP regulation. <br /> The five-year anniversary date for the next PHA and revalidation is July 9, 2025 as <br /> determined by this report, which servers as documentation of the completion of the <br /> PHA (per CalARP Section 2755.2 and 2760.2) <br /> 4 <br />
