The plan was last submitted to CERS on <br /> 4/6/2017. An annual submittal was not made for 2018, 2019, and 2020. In addition,the uploaded plan had the <br /> incorrect phone numbers for the San Joaquin County Environmental Health Department and San Joaquin County <br /> Office of Emergency Services. <br /> A business plan shall include the following emergency response procedures for a release or threatened release of <br /> hazardous materials, including, but not limited to,the following: <br /> -immediate notification of local emergency personnel and the Environmental Health Department(EHD)at <br /> 209-468-3420 <br /> -procedures for the mitigation of a release or threatened release to minimize any potential harm or damage to <br /> persons, property, or the environment <br /> -evacuation plans and procedures, including immediate notice,for the site <br /> Immediately log into the California Environmental Reporting System (CERS)at http://cers.calepa.ca.gov/, upload the <br /> correct or updated information, and submit to the EHD for approval. <br /> This is a repeat violation, Class II. <br /> 8 HSC 25505(a)(4) Failed to establish and electronically submit a reasonable or an adequate training program. <br /> The training program was last submitted to CERS on 4/6/2017. An annual submittal was not made for 2018, 2019, <br /> and 2020. <br /> Training programs for all employees and annual training, including refresher courses, shall include familiarity with <br /> the facility's Emergency response plans and procedures in the event of a release or threatened release of a <br /> hazardous material.Training shall cover, but not limited to, all of the following: <br /> (A) Immediate notification contacts to the appropriate local emergency response personnel and to the unified <br /> program agency. <br /> (B)Procedures for the mitigation of a release or threatened release to minimize any potential harm or damage to <br /> persons, property, or the environment. <br /> (C) Evacuation plans and procedures, including immediate notice,for the business site. <br /> These training programs may take into consideration of each employee. Immediately prepare or update the facility's <br /> training program, log into the California Environmental Reporting System (CERS)at http://cers.calepa.ca.gov/, <br /> upload the correct or updated information, and submit to the EHD for approval. <br /> This is a repeat violation, Class II. <br /> 9 HSC 25508(a)(1) Failed to annually review and certify that the HMBP is complete and accurate by the due date. <br /> The business plan information has not been reviewed and resubmitted in the California Environmental Reporting <br /> System (CERS)annually. The last submittal was made on 4/6/2017. An annual submittal was not made for 2018, <br /> 2019, and 2020. The hazardous materials inventory shall be submitted by January 15 of each calendar year and <br /> may be submitted beginning November 1 of the previous year.
