Spillage of a green solidified substance was observed on the concrete pad beneath the electrostatic precipitators. <br /> The substance was coming out of a pipe connected to the electrostatic precipitators (see photos IMG 6889-90). Bill <br /> Boscacci, EH&S Director, stated this substance would be cleaned up and placed in the roll-off bin used for <br /> non-hazardous waste batch,without first determining if the green substance is hazardous. <br /> 2.An off-white substance was found on the floor in the vicinity of the facility's newest generator(IMG 6932). <br /> Any person who generates a waste shall determine if the waste is a hazardous waste. Immediately stop disposal of <br /> the green substance from the electrostatic precipitators, make a hazardous waste determination for the substance, <br /> and manage it according the Title 22 hazardous waste regulations. <br /> Submit a statement and supporting documentation explaining how this waste was managed. <br /> This is a repeat violation, Class II. <br /> 111 CCR 66265.54 Contingency plan not current. <br /> At the time of inspection, Bill Boscacci, noted that the contingency plan did not list the street sweeper on the <br /> emergency equipment list. The street sweeper was observed cleaning the asphalt outside near the electrostatic <br /> precipitators, at the time of inspection. <br /> Bill also noted that the location of the vacuum listed on the emergency equipment needed to be updated, as a vacuum <br /> was also being used in the Batch House, in addition to the Mold Shop. <br /> Bill updated the contingency plan accordingly at the time of inspection. <br /> The contingency plan shall be reviewed and immediately amended, if necessary,whenever: <br /> -applicable regulations are revised; <br /> -the plan fails in an emergency; <br /> -facility changes in design, construction, operation, maintenance, or other circumstances that increases the <br /> potential for fires, explosions, releases of hazardous waste, or changes the response necessary in an emergency; <br /> -the list of emergency coordinators changes; or <br /> -the list of emergency equipment changes. <br /> Immediately amend the contingency plan, and submit a copy to the EHD for review. <br /> This was corrected on site. <br /> This is a repeat violation, Class II. <br /> FA0006674 PR0220086 SCO01 06/05/2019 <br /> EHD 22-03 Rev.09/27/2018 Page 5 of 13 RCRA Large Quantity Hazardous Waste Generator OIR <br /> 1868 E.
