bond <br /> • Tank supports <br /> • Tank foundation and external supports <br /> • Tank gauges and alarms <br /> • Tank appurtenances <br /> • Emergency vents <br /> • Release prevention barriers <br /> • Spill Control systems <br /> The inspection of the Poly Tank was performed based on procedure and scope outlined in the New <br /> York State Department of Environmental Conservation document DER-16: Five-Year Inspection of <br /> Plastic Tanks. There is no widely accepted industry standard for polyethylene tank inspections, and <br /> this standard was used because it presents detailed procedures for performance of a thorough <br /> external inspection for plastic tanks. <br /> 4. Certification of the Poly Tank System <br /> Upon inspection, Ms. Karen Petryna (ID#AST-2072), a certified STI SP001 tank inspector, has <br /> found the Poly Tank System to be in good condition with no significant corrosion, cracks, or leaks <br /> and that ancillary equipment is supported and protected against physical damage and excessive <br /> stress due to settlement, vibration, expansion, or contraction. Based on this and the information <br /> provided in this Engineering Assessment Report, as required by 22 CCR 66265.192, the Poly Tank <br /> System has been found to have sufficient structural integrity and is acceptable for the transferring of <br /> hazardous waste. As described herein, the Poly Tank System complies with all applicable elements <br /> of 22 CCR 66265.190 through 200. <br /> The Inspector's STI Certification is presented in Appendix A. <br /> 5. Poly Tank System Description <br /> The Poly Tank System is located at the approximate center of the Site and Plant, approximately 75 <br /> feet north of the Main Gate. It sits on a concrete mat foundation on the ground surface in an open <br /> area adjacent to the machine parts cleaning room. Four-inch diameter chlorinated polyvinyl chloride <br /> (CPVC) pipe connects the drain from the hot vat and steam cleaning room on the second story of <br /> the adjacent building to the inlet on the top of the Poly Tank such that the oily water and sodium <br /> hydroxide cleaning solution wastewater flows by gravity into the Poly Tank. CPVC is significantly <br /> more flexible and can withstand higher temperatures than standard PVC. <br /> The CPVC conveyance piping is secured to the building with metal clamps. The outlet from the Poly <br /> Tank, where the wastewater is drained out for offsite disposal, is located near the tank base. The <br /> liquid level in the Poly Tank can be monitored through a digital display of the liquid level sensor <br /> located on the wall of the adjacent building. <br /> GHD I Engineering Assessment and Certification of Integrity of the 1,550-Gallon Polyethylene Waste Tank System 111189093(2)1 Page 2 <br />
