Maintenance and monitoring records for the last three years were not found on site. -The 2018 monitoring system certification test results were not found on site. -No maintenance records or alarm logs were found for the L3 (91 STP sump sensor)fuel alarm on 7/13/18. These records shall be maintained on site for at least three years. Monitoring records include: (1) date and time of all monitoring or sampling; (2) monitoring equipment calibration and maintenance records; (3) results of any visual observations; (4) results of sample analysis performed a lab or in the field; (5) logs of all readings of gauges or other monitoring equipment, ground water elevations, or other test results; (6) results of inventory readings and reconciliations. Immediately locate and maintain all missing maintenance and monitoring records for the last three years on site and submit copies to the EHD. This is a minor violation. 202 CCR 2715(c)(3), 2715(c)(4) Failure to maintain the "Facility Employee Training Certificate" on-site (after October 1, 2018). -The Facility Employee Training Certificate dated 4/8/19 contained no CERS ID, no city, no zip code, no designated operator(DO) mailing address, and no phone number. -The Facility Employee Training Certificate dated 3/18/19 contained no CERS ID, no city, no zip code, no DO mailing address, no phone number, no business name, no date of training, and no "date of assuming responsibility as a facility employee"for three of the four employees trained. -The Facility Employee Training Certificate dated 12/6/18 contained no CERS ID and no phone number. -The DO training conducted on 12/13/18 was not documented on its own form; it was placed on the 12/6/18 training form. All site employees must be trained prior to taking responsibility as a facility employee if after October 1, 2018, or within 30 days of hire if prior to October 1, 2018. The Facility Employee Training Certificate shall contain accurate information for all employees trained. Update the Facility Employee Training Certificates to contain the accurate information and submit copies of the updated form to the EHD. This is a minor violation. 204 CCR 2712(1) Current monitoring plan approved by the EHD not found on site. An approved copy of the monitoring plans were not available for inspection. A copy of these plans shall be accessible on site at all times. Immediately locate and retain a copy on site. This is a minor violation.
