All floats <br /> were removed and replaced with a single sensor. The sensor in each UDC is placed in one of the open cups, <br /> leaving the other two cups unmonitored and the sensor not located to detect a leak at the earliest opportunity. The <br /> liquid in UDC 5/6 filled most of one cup while the sensor was located in a different, dry cup. Monitoring equipment <br /> shall be maintained to be able to detect a leak at the earliest possible opportunity. Correct immediately by having a <br /> properly licensed, trained, and certified contractor replace or repair the component(LG 113-x listed, if applicable) <br /> under permit and inspection from the EHD. <br /> This is a Class II violation. <br /> 315 HSC 25291(e)Water in secondary containment not removed, analyzed, and properly disposed of (pre-Jul 2003). <br /> Liquid was observed in UDCs 1/2, 5/6, and 7/8. If water could enter into the secondary containment by precipitation <br /> or infiltration, it must be removed and disposed of properly. Immediately remove this liquid, make a hazardous <br /> waste determination per Title 22 hazardous waste regulations, and manage it accordingly. Ensure that the UDCs <br /> are maintained free of liquid. <br /> This is a repeat violation, Class II. <br /> 319 CCR 2635, 2637.1, 2665, 2712 No spill bucket, failed to contain 5 gallons, no means to keep it empty, or failed to be <br /> tested. <br /> The 87 spill container failed when tested. All spill containers shall have a minimum capacity of five gallons and be <br /> capable of containing a spill or overfill until it is detected or cleaned up. The technician replaced the o-ring, retested <br /> the spill container, and it passed. <br /> This was corrected on site. <br /> This is a minor violation. <br /> 320 CCR 2635, 2637.2, 2665, 2712 Failure to comply with the installation, inspection, or record keeping of the overfill <br /> prevention. <br /> Overfill prevention testing has not been conducted. According to a work order dated 9/26/18, testing was <br /> attempted, but could not be conducted. This testing was required to be conducted by October 13, 2018. <br /> Immediately schedule this testing and provide 48 hours notification to the EHD. Ensure all future testing is <br /> conducted within required time frames. <br /> This is a Class II violation. <br /> Overall Inspection Comments: <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> documenting the corrective actions that have been or will be taken for each violation, and any supporting <br /> paperwork, by May 16, 2019. <br /> Starting September 1, 2018, all in-office CERS help will be provided at EHD hourly rate ($152).
