A hazardous waste generator shall not treat, <br /> store,dispose of,transport or offer for transportation, hazardous waste without a hazardous waste ID number. Go <br /> to the DTSC web site https://dtsc.ca.gov/apply-for-hazardous-waste-epa-id-number/to reactivate the ID number. <br /> Immediately reactivate your ID number and submit evidence to the EHD. <br /> This is a minor violation. <br /> 114 HSC 25160.2(b)(4)Consolidated manifest receipts failed to contain complete information. <br /> The consolidated manifest receipts for used oil from American Valley Waste Oil dated 3/5/19, 6/15/19& 11/21/17 <br /> were missing the correct ID number(CAL000348389). Consolidated receipts used the ID number CAL000254598 <br /> which is assigned to the location at 25451 Mountain House Parkway in Tracy. All copies of each receipt shall <br /> contain all of the following information: <br /> -The name, address, identification number, contact person, and telephone number of the generator, and the <br /> signature of the generator or the generator's representative; <br /> -The date of the shipment; <br /> -The manifest number; <br /> -The volume or quantity of each waste stream received, its California and RCRA waste codes,the waste stream <br /> type listed in subdivision (c), and its proper shipping description, including the hazardous class and United <br /> Nations/North America(UN/NA)identification number, if applicable; <br /> -The name, address, and identification number of the authorized facility to which the hazardous waste will be <br /> transported; <br /> -The transporter's name, address, and identification number; <br /> -The driver's signature; <br /> -A statement, signed by the generator, certifying that the generator has established a program to reduce the <br /> volume or quantity and toxicity of the hazardous waste to the degree, as determined by the generator,to be <br /> economically practicable. <br /> Ensure that all future receipts for hazardous waste shipments using a consolidated manifesting procedure contain all <br /> required information. <br /> This is a minor violation. <br /> 303 CCR 66262.34(d)(2) Failed to maintain all communication or alarm systems, spill control, or decontamination <br /> equipment. <br /> Two yellow secondary containment pallets located in the hazardous waste storage area were observed with oily <br /> sludge in the bottom. All facility communications or alarm systems,fire protection equipment, spill control equipment, <br /> and decontamination equipment,where required, shall be tested and maintained as necessary to assure its proper <br /> operation in time of emergency.
