The following is an itemized list of hazardous waste violations that have not been <br /> addressed for MAIN STREET SMOG as of September 11, 2019. <br /> Open violations from October 112018 inspection <br /> Violation #102 -Failed to determine if a waste is a hazardous waste. <br /> Observed that this facility is using absorbent for spills. The facility is unable to provide disposal record for used <br /> absorbent. <br /> Any person who generates a waste shall determine if the waste is a hazardous waste. <br /> Once spill absorbent cannot be reused, make a hazardous waste determination for the used absorbent, and <br /> manage it according the Title 22 hazardous waste regulations. Submit a statement and supporting documentation <br /> explaining how this waste will be managed. <br /> ❑ This violation was corrected ❑ This violation will be corrected by (date): <br /> ❑ Supporting documents included <br /> Describe actions taken or will be taken to correct violation: <br /> Violation #104-No modified contingency plan. <br /> Facility failed to prepare a modified contingency plan. <br /> The owner or operator shall prepare a contingency plan designed to minimize hazards to human health or the <br /> environment from fires, explosions, or any unplanned release of hazardous waste to air, soil, or surface water and <br /> immediately implement that plan whenever there is a fire, explosion, or release of hazardous waste which could <br /> threaten human health or the environment. <br /> Immediately prepare a modified contingency plan and submit a copy to the EHD. <br /> ❑ This violation was corrected ❑ This violation will be corrected by (date): <br /> ❑ Supporting documents included <br /> Describe actions taken or will be taken to correct violation: <br /> Violation #105 -No emergency coordinator. <br /> An emergency coordinator and modified contingency plan information is lacking. <br /> There must be at least one emergency coordinator on site or on call to coordinate emergency response measures, <br /> and the following information must be posted by a phone: the name and phone number of the emergency <br /> coordinator; location of fire extinguishers, spill control equipment, and if present, fire alarm; and the phone number <br /> of the fire department, unless the facility has a direct alarm. <br /> Immediately appoint an emergency coordinator and post the modified contingency plan by a phone.
