SAN _10 A Q U I N Environmental Health Department <br /> COUNTY <br /> Small Quantity Hazardous Waste Generator Inspection Report <br /> Facility Name: Facility Address: Eat <br /> DOLLAR TREE#07648 318 W MAIN ST RIPON 15 2019 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> Renesha placed the above mentioned items in their appropriate HW totes(except the Card Reader Cleaning Card). <br /> Renesha placed the unlabeled used paper towels in a CB HW tote, and the leaky energy drinks into an STOU HW <br /> tote. <br /> -Used paper towels were found inside a bag labeled"Stericycle"and"One Waste Per Bag,"located at register 2 at <br /> the front of the store. Renesha stated employees place the paper towels, used with Fabuloso or window cleaner to <br /> clean the facility, into the bag, and once the bag is full it is placed in a HW tote. Renesha placed the bag of paper <br /> towels in a CB HW tote at the time of inspection. (IMG 6444,6446-9) <br /> All hazardous waste containers shall be marked with the following information: <br /> -the words"Hazardous Waste' <br /> -name and address of generator <br /> -hazardous properties <br /> -physical state <br /> -composition(contents) <br /> -accumulation start date <br /> Immediately place the Card Reader Cleaning Card in a properly labeled hazardous waste container and ensure that all <br /> hazardous waste containers are marked with all the required information. <br /> This is a Class II violation. <br /> Overall Inspection Comments: <br /> Notes: <br /> -Violation 605 was removed. <br /> An inspection checklist was provided to the facility operator on the day of inspection. The EHD has written and <br /> amended the complete report which replaces the initial checklist on 6/24/2019. <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> documenting the corrective actions that have been or will be taken for each violation, and any supporting <br /> paperwork, by 7/24/2019. <br /> Starting September 1, 2018, all in-office CERS help will be provided at EHD hourly rate ($152).
