Main Street <br /> Ripon, CA 95366 <br /> Dear Inspector Florido, <br /> In response to the inspection conducted on May 15, 2019, and amended June 24, 2019, at Store <br /> #07648 in Ripon, California,please see the outline below, and the exhibits which follow, <br /> referencing existing procedures, documentation and/or actions taken: <br /> • The Card Reader cleaning card was determined to be hazardous waste. It was placed in <br /> the Aerosols and Flammables tote which was picked up by Stericycle on June 28, 2019. <br /> See the generator manifest(EXHIBIT A). <br /> • An Environmental Health& Safety Specialists retrained the Store Manager and three <br /> Assistant Store Managers on the hazardous waste program and related emergency <br /> response procedures on June 28, 2019 (EXHIBIT B). A copy of the training guide used is <br /> provided as an email attachment. <br /> • Generator manifests are submitted to DTSC on the Company's behalf by our hazardous <br /> waste vendor Stericycle (EXHIBIT Q. However, generator copies of the manifests cited <br /> in the report were also resubmitted to DTSC by Dollar Tree (EXHIBIT D). <br /> The signed certification is also included.
