Renesha stated that the items in this tote are waiting to be processed, and once processed are placed in <br /> the proper HW tote. Renesha said she processes the items in the tote one or two times each month. Renesha also <br /> stated the accumulation of the items found in the tote started about one month ago(IMG 6382-3,6388,6412-3, <br /> 6416-7): <br /> o*An open package labeled as Card Reader Cleaning Card and with the statement that the item is saturated with <br /> isopropanol. Renesha stated the item is used by the facility to clean the card payment readers at the cash registers. <br /> The item did not have a barcode that could be scanned. Renesha stated she did not know which HW tote the item <br /> should be placed in. Renesha left the package in the tote. However, according to the Dollar Tree Potentially <br /> Hazardous Materials Management poster, posted in the HW storage area, items containing isopropyl or IPA are <br /> classified as Aerosols, Flammables, &Combustibles(AF). <br /> o An unsealed bottle of Assured Blue Mint Mouth Rinse. Renesha placed the mouth rinse in a bag labeled HW. <br /> The scanner stated the mouth rinse was to be placed in an AF HW tote. <br /> Any person who generates a waste shall determine if the waste is a hazardous waste. <br /> Immediately stop storing possible hazardous waste that is not labeled, make a hazardous waste determination for the <br /> Card Reader Cleaning Card, and manage it according the Title 22 hazardous waste regulations. <br /> Submit a statement and supporting documentation explaining how this waste was managed. <br /> This is a Class II violation. <br /> FA0000734 PRO543984 SC001 05/15/2019 <br /> EHD 22-01 Rev.09/27/2018 Page 4 of 17 Small Quantity Hazardous Waste Generator QIR <br /> 1868 E.
