Immediately log into CERS, update <br /> the required information, and submit for review by the EHD. <br /> This is a Class II violation. <br /> 115 CCR 2715(a)(1)(B), 2715(a)(3)"Designated Underground Storage Tank Operator Identification Form" not submitted <br /> within 30 days. <br /> Zane Nimmo is performing designated operator inspections at the facility and is not listed on the submitted <br /> statement of designated operators. Notification was not provided to the EHD within 30 days of the change. Any <br /> changes shall be reported to the EHD within 30 days. A <br /> copy of the test report must be submitted within 30 days of the tests. Take all necessary precautions to ensure <br /> testing and submittal of test reports are performed in a timely manner. <br /> This is a minor violation. <br /> 201 CCR 2712(b) Failure to maintain monitoring, maintenance, and follow up action records. <br /> Overfill prevention test records for 8/29/18 were not found on site. These records shall be maintained on site for at <br /> least three years. Monitoring records include: (1)date and time of all monitoring or sampling; (2) monitoring <br /> equipment calibration and maintenance records; (3) results of any visual observations; (4) results of sample analysis <br /> performed a lab or in the field; (5) logs of all readings of gauges or other monitoring equipment, ground water <br /> elevations, or other test results; (6) results of inventory readings and reconciliations.
