Please see response to Item No. 105 for more information on Cooks ^���^T.•��,T <br /> Collision's training. <br /> Cooks Collision has contracted with a licensed hazardous waste disposal company, <br /> Safety-Kleen Systems,Inc., to ensure proper disposal of hazardous waste accumulated at <br /> the facility. Cooks Collision has material safety data sheets for all hazardous materials <br /> used at the facility,waste manifests and waste detenninations for all hazardous waste <br /> removed by its hazardous waste disposal vendor. Copies of the December 11,2014,and <br /> December 24,2014, Uniform Waste Manifests and associated waste determinations for <br /> waste paint removed from the facility are enclosed as Attachment B. <br /> Item No. 103: CCR 66262.34(d)(2). No k1odified Contingency Plan. An emergency <br /> coordinator and modified contingency plan information is lacking. There must beat <br /> least one emergency coordinator on site or on call to coordinate emergency response <br /> measures, and the following information must be posted by a phone: the name and phone <br /> number of the emergency coordinator-; location offline extinguishers, spill conh•ol <br /> equipment, and if present,fire alarm; and the phone number•of the fire department, <br /> unless the facil ity has a direct alarm. Immediately appoint an emergency cool•dinator <br /> and post the required information by a phone. A form is provided that carr be used for <br /> this pun pose. Class II violation. <br /> Response to Item No. 103: <br /> The Emergency Coordinators for the facility are Victor Lima and Adam Carl. The <br /> contact information for W. Lima and Mr. Carl is posted at each phone and each exit <br /> throughout the facility and at the hazardous waste accumulation area. Additionally, all <br /> emergency communications information, including phone numbers for the local fire <br /> department, is posted at these locations. A copy of the posting is enclosed for your <br /> review in Attachment C. In addition to the emergency information, Cooks Collison posts <br /> copies of the Hazardous Materials Business Plan Site Map that indicates the precise <br /> locations of all emergency equipment at the facility. The Site Map is posted in the break <br /> room and at each exit. A copy of the Site Map is enclosed as Attachment D. <br /> Itent No. 105: CRC66262.34(d)(2).
