However, in the California <br /> Environmental Reporting System (CERS)the facility name is Family Dollar#12078. Correct this discrepancy in <br /> the facility name and notify the EHD once completed. <br /> -The amount of hazardous waste generated in the last calendar year could not be determined due to manifests <br /> not being readily available for review at the time of inspection. Also, there were no manifests found on file with <br /> the Department of Toxic Substances Control (DTSC)at the time of inspection. <br /> -According to Michael Griffin, store manager, the facility had a hazardous waste pick-up before the 2018 <br /> holiday season, and another pick-up which was more recent. At the time of inspection, only 8-10 hazardous <br /> waste items were observed stored in the hazardous waste totes, as mentioned in the above violations. Michael <br /> stated that many items are salvaged and are sold at a marked down price to reduce the amount of hazardous <br /> waste generated. However, Michael also stated empty containers that previously held liquid chemicals and the <br /> spill clean-up waste from such chemicals are disposed of in the trash. Provide a statement to the EHD <br /> explaining Family Dollar's process for determining which items may be salvaged, specifically potential hazardous <br /> waste items. <br /> -According to Michael and Gloria Oros, district manager, SPAR, a third party group, takes recalled items from <br /> the facility and mails the items via UPS or FedEx to either the facility's corporate or distribution center. Gloria <br /> stated she did not know where exactly the items are mailed to. The following documentation and items were <br /> found in the facility's office: <br /> - Family Dollar documentation noting the destruction of nine items of.125 fl. oz. of Baby Orajel from the facility <br /> by a SPAR representative, Paul Cesar, on 6/12/18 due to a voluntary recall (see photo IMG 4939-4940). <br /> - Family Dollar documentation dated 10/4/2018 noting the immediate isolation and hold of 24 HR Prevacid <br /> Capsule 14 CT (SKU 914127)for a third party, and not to be sold due to a quality concern. This documentation <br /> also notes possible disposal of this item. A Family Dollar plastic bag was found containing approximately 5-10 <br /> boxes of 24HR Prevacid Capsule 14 CT. (IMG 4942-4945) <br /> - Lastly, Family Dollar documentation dated 1/31/2019 noting the immediate isolation and hold of Crocs <br /> Lighters 5pk (SKU 900836)and Cres Sprkwhl Cgrett Lghtr 3pk (SKU 900870), and not to be sold due to a <br /> quality concern. This documentation notes that the product should be stored until further notice. Four Crocs <br /> Lighters 5pck, labeled as containing "extremely flammable gas under pressure,"were found in a box.
