SAN J O A Q U I N Environmental Health Department <br /> —COUNTY <br /> Small Quantity Hazardous Waste Generator Inspection Report <br /> Facility Name: Facility Address: Date: <br /> REVCHEM COMPOSITES 4807 S AIRPORT WAY, STOCKTON ]August 20, 2019 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS ll,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 102 CCR 66262.11 Failed to determine if a waste is a hazardous waste. <br /> OBSERVATIONS: The facility has Central Valley Lift Truck, Inc., service the forklifts and take the waste back with <br /> them to their shop. According to invoice 46611 (3/29/2019),46118(1/18/2019),and 46110(1/15/2019), used <br /> antifreeze waste was taken off site by the service company on a non-uniform or consolidated manifest. The facility <br /> did not have any waste antifreeze on site. <br /> REGULATION: Any person who generates a waste shall determine if the waste is a hazardous waste as specified <br /> in Title 22 California Code of Regulations(CCR). There are wastes that are listed as hazardous wastes. There are <br /> wastes that exhibit one or more of the hazardous waste characteristics:toxic, corrosive, reactive or ignitable. <br /> CORRECTIVE ACTION: Immediately cease disposal of the antifreeze, make a hazardous waste determination, <br /> and start to manage it according to Title 22 CCR. Use Safety Data Sheets(SDS),waste sampling and test results <br /> or other knowledge to support your hazardous waste determination. Waste testing must be done using methods <br /> specified in Title 22 CCR including sections 66261.20-24. Submit a statement and supporting documentation with <br /> your hazardous waste determination on how the used antifreeze will be handled in the future. <br /> This is a minor violation. <br /> Overall Inspection Comments: <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> documenting the corrective actions that have been or will be taken for each violation, and any supporting <br /> paperwork, by September 19, 2019. <br /> Starting September 1, 2018, all in-office CERS help will be provided at EHD hourly rate ($152). To schedule an <br /> appointment, please call (209) 468-3420. <br /> NOTES: <br /> The following minor violation was corrected during the inspection: <br /> OBSERVATION: A complete modified contingency plan was not posted at the facility. The contingency plan was <br /> missing the emergency coordinator information. <br /> REGULATION: The business operator must post the following information next to telephones or in areas directly <br /> involved in the generation and accumulation of hazardous waste: the name and emergency telephone number of <br /> the emergency coordinator, the location of fire extinguishers and spill control material, and the telephone number of <br /> the fire department unless the facility has a direct alarm. <br /> The contingency plan was corrected on site.
