Martinez (no signature indicating training not completed) <br />- Kassandra stated that she received hazardous waste computer-based training once when she was first hired by <br />Family Dollar, over a year ago, but has not received hazardous waste training since then. Kassandra explained that <br />she needs hands-on in-person training, in order to help her retain the training information. <br />The generator must ensure that all employees who handle hazardous waste are thoroughly familiar with proper <br />waste handling and emergency procedures. <br />Provide proof of re-training and training materials to the EHD for employees whose responsibilities include <br />hazardous waste. <br />This is a Class II violation. <br /> 109 CCR 66262.23(a)(4) Failed to send generator manifest copies to DTSC within 30 days. <br />According to Kassandra Witthoft, store manager, the facility does not send generator copies of manifests to DTSC. <br />Generator copies of the following manifest numbers were not sent to DTSC within 30 days of shipment of <br />hazardous waste: <br />011385056FLE (9/11/2018) (IMG 5153) <br />011385055FLE (9/11/2018) (IMG 5152) <br />At the time of inspection, Kassandra stated that the last hazardous waste pick-up was about 1 week ago, however <br />the generator copy of the manifest was not readily available for review. <br />The generator manifest copy shall be submitted to DTSC for every shipment on a manifest when California is either <br />the generator state or the destination or consignment state. The generator manifest copy shall be mailed to: <br /> DTSC Generator Manifests <br /> P.O.
