Environmental Health Department <br />Small Quantity Hazardous Waste Generator Inspection Report <br />Date: <br />April 25, 2019 <br /> Facility Address: <br /> 2318 S AIRPORT WAY, STOCKTON <br /> Facility Name: <br /> FAMILY DOLLAR #31867 <br />SUMMARY OF VIOLATIONS <br />(CLASS I, CLASS II, or MINOR - Notice to Comply) <br />RemarksItem # <br /> 202 HSC 25189.5(a) Disposed or caused disposal of HW at an unauthorized point. <br />- Two containers of Ampro Pro Styl protein styling gel were found in a cardboard box behind a register at the front of <br />the facility. One of the containers was broken and leaking, contaminating the box and the second container (IMG <br />5600-5603). Later, the inspector found the contaminated cardboard box and the containers of Ampro Pro Style protein <br />styling gel in a trash can located behind the register (IMG 5630, 5632). When asked who placed the items into the <br />trash, Monica stated she did not know. When the containers of gel were scanned, the hand held scanner indicated the <br />gel should be placed in a ST HW tote (IMG 5636). Monica removed the two containers of gel and the contaminated <br />cardboard box from the trash and took them to the HW storage area. <br />- A full bottle of Body Fantasies Coconut Fragrance Body Spray was found missing the actuator and in a trash can <br />located behind a register at the front of the facility (IMG 5630-1, 5633). Monica did not know who placed the item in <br />the trash. The hand held scanner indicated Body Fantasies body spray should be placed in the AF HW tote (IMG <br />5634). Monica removed the body spray from the trash and took it to the HW storage area. <br />- A full bottle of Modesa Vitamin Moisturizing Skin Oil was found in a trash can located behind a register at the front of <br />the facility (IMG 5630-1, 5633). Monica did not know who placed the item in the trash. The hand held scanner <br />indicated the Modesa skin oil should be placed in the CA HW tote (IMG 5635). Monica removed the skin oil from the <br />trash and took it to the HW storage area. <br />Hazardous wastes shall be disposed of only by transportation to a permitted hazardous waste treatment, storage, and <br />disposal facility (TSDF). <br />Immediately cease disposal of hazardous waste to the trash and ensure that all future waste is hauled by a licensed <br />hazardous waste transporter to a permitted TSDF. <br />Submit a statement to the EHD explaining how the above mentioned waste that was found in the trash was managed. <br />This is a Class I violation. <br /> 301 CCR 66262.34(d)(2) Facility not maintained to minimize the release of a hazardous waste. <br />A sales shelf was found with spillage of what appeared to be Ajax with Bleach powder cleaner (IMG 5606). The <br />spillage was also on the floor in front of the shelf. The sales tag on the shelf read "Ajax powder cleanser 28oz <br />14-12099 05364" (IMG 5605). <br />Facilities shall be maintained and operated to minimize the possibility of a fire, explosion, or release of hazardous <br />waste to air, soil, or surface water which could threaten human health or the environment. <br />The Ajax spill was cleaned up, placed in a bag labeled "ST," and into an ST HW tote by a store employee at the time <br />of inspection. <br />This was corrected on site. <br />This is a Class II violation. <br />Page 9 of 16 <br />FA0001729 PR0544373 SC001 04/25/2019 <br />EHD 22-01 Rev. 09/27/2018 Small Quantity Hazardous Waste Generator OIR <br />1868 E.
