The EHD has determined that the property noted above has been <br /> contaminated by methamphetamine laboratory activities and, in accordance with <br /> California Health and Safety(H&S) Code Section 25400.22(a)(2), is issuing the attached <br /> order prohibiting the use or occupancy of the property until such time that all <br /> investigative and remedial actions are complete. <br /> For your information, the following check list is provided as guidance on the submittals <br /> and work required pursuant to H&S Code 25400.10 et. seq. <br /> ❑ Submittal of the attached Demonstration of Retention of Authorized Contractor form within 30 <br /> days of receipt of this letter and Order. <br /> ❑ Submittal of the Preliminary Site Assessment Work Plan within 30 days after the submittal of the <br /> Demonstration of Retention of Authorized Contractor form. <br /> ❑ Performance of the work outlined in the approved Preliminary Site Assessment Work Plan. All <br /> remediation work must be completed within 90 days after the approval of the work plan. <br /> ❑ Submittal of the Preliminary Site Assessment Report describing the scope of work performed and <br /> the results of the post remediation sampling. <br /> All enforcement and oversight costs incurred by the EHD are the responsibility of the <br /> owner(H&S Code Section 25400.46).
