As the requirements of California Health <br /> and Safety Code Sections 25400.36 and 25400.38 have been met and the scope of work <br /> has been approved, the work can commence at the site. Please be advised that the <br /> following conditions apply: <br /> 1.0 Provide information demonstrating to the EHD that the contractor to perform the work <br /> at the site is qualified prior to the start of work at the site. <br /> 2.0 Rooms/items in the house currently not being proposed for sampling as part of this <br /> work plan may be required to be sampled later, based on the results of the current <br /> proposed sampling. <br /> 3.0 Only work approved in the work plan should be performed.
