Immediate <br /> notification contacts to the appropriate local emergency response personnel and to the unified program agency; 2. <br /> Procedures for the mitigation of a release or threatened release to minimize any potential harm or damage to <br /> persons, property, or the environment; and 3. Evacuation plans and procedures, including immediate notice, for the <br /> business site; was not available at the time of inspection. The business plan shall include provisions for ensuring <br /> that appropriate personnel receive initial and annual refresher training. All employees shall be trained in safety <br /> procedures in the event of a release or threatened release of a hazardous material, including, but not limited to, <br /> familiarity with the following plans and procedures: <br /> (A) Immediate notification contacts to the appropriate local emergency response personnel and to the unified <br /> program agency. <br /> (B) Procedures for the mitigation of a release or threatened release to minimize any potential harm or damage to <br /> persons, property, or the environment. <br /> (C) Evacuation plans and procedures, including immediate notice, for the business site. This training shall be <br /> documented electronically or by hard copy and shall be made available for a minimum of three years. <br /> Immediately provide employee training for appropriate personnel and submit a copy of the training records to the <br /> EHD. <br /> This is a Class II violation. <br /> Overall Inspection Comments: <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> documenting the corrective actions that have been or will be taken for each violation, and any supporting <br /> paperwork, by September 5, 2020. <br /> To minimize person to person contact EHD is choosing to write the name of person receiving the report instead <br /> of having them sign. Starting September 1, 2018, all in-office CERS help will be provided at EHD hourly rate <br /> ($152).
