OBSERVATION: Financial responsibility documents have not been submitted to the EHD. The most recent financial responsibility documents in CERS are dated 08/20/2019. REGULATION GUIDANCE: Current financial responsibility documents are required to be submitted annually. CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, and upload the required documents. This is a minor violation. 107 CCR 2632(d), 2641(h), 2711(a) Plot plan/site map not submitted or failed to completely show where monitoring is performed. OBSERVATION: An accurate UST Monitoring Site Plan was not submitted. Missing Piping layout and all sensors location. REGULATION GUIDANCE: A site plan must be submitted identifying the locations where monitoring will be performed. The site plan must show the general layout of tanks and piping and clearly identify locations of the following equipment, if installed: 1) monitoring system control panels; 2) in-tank liquid level probes (if used for leak detection); 3)devices monitoring tank annular spaces or vault; 4)devices monitoring product piping; 5)devices monitoring fill piping; 6) devices monitoring vent piping; 7)devices monitoring vapor recovery piping; 8) devices monitoring vent/transition sumps; 9)devices monitoring under-dispenser containment; 10) line leak detectors; and 11) devices monitoring any other secondary containment areas. CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/and upload a copy of the UST Monitoring Site Plan. This is a minor violation. 113 CCR 2638(c), 2638(d) Failure to submit"Monitoring System Certification Form"within 30 days of completion of the test. OBSERVATION: Annual monitoring system certification, leak detector testing, was performed on 08/20/2019 and a test report was submitted on 10/30/2019, one month late. REGULATION GUIDANCE: A copy of the test report must be submitted within 30 days of the tests. CORRECTIVE ACTION: Take all necessary precautions to ensure testing and submittal of test reports are performed in a timely manner. This is a minor violation.
