The report was amended <br /> and replaces the initial report. Onsite for a routine (post installation) monitoring system certification and a routine <br /> Underground Storage Tank(UST) inspection. All passed. This facility is not approved to operate please see <br /> SR0079417 inspection report dated 7/7/2020. The following items need to be addressed: <br /> - Submit and maintain complete and current financial responsibility documents. <br /> -The UST Facility Operating Permit Application contains an invalid California Department of Tax and Fee <br /> Administration (CDTFA) Underground Storage Tank Account Number. The CDTFA number given during the <br /> service request inspection was 238560640. However, CDTFA number 255886720 was observed in CERS. <br /> -The monitoring plans incorrectly indicate "no"for training. <br /> -The monitoring plans do not indicate the sensors performing monitoring. <br /> -The monitoring plans are missing information under"Name of Second Person Having Responsibility." <br /> -A UST Monitoring Site Plan was not submitted. <br /> -The Designated Underground Storage Tank Operator Identification Form was not found in CERS. <br /> -The Underground Storage Tank Statement of Understanding and Compliance Form was not found in CERS. <br /> -An approved copy of the monitoring plans were not available for inspection. <br /> -An approved copy of the response plan was not available for inspection. <br /> -The designated operator employee training has not been performed. <br /> To minimize person to person contact EHD is choosing to write the name of person receiving the report instead <br /> of having them sign. Starting September 1, 2018, all in-office CERS help will be provided at EHD hourly rate <br /> ($152).
