Time In: 11:30 am <br /> Time Out: 12:15 om <br /> Food Program Complaint Inspection Report <br /> Name of Facility: FORUM DYNASTY RESTAURANT Date: 09/28/2020 <br /> Address: 1264 W LATHROP RD, MANTECA 95336 <br /> Owner/Operator: FORUM DYNASTY Telephone: (209)239-2399 <br /> Program Element: 1600-FOOD PROGRAM Complaint#: C00052683 <br /> DESCRIPTION OF COMPLAINT ALLEGATIONS <br /> The San Joaquin County Environmental Health Department received the following complaint: <br /> COMPLAINANT PLACED AN ORDER OF MU SHU PORK FOR TAKE OUT. COMPLAINANT ALLEGES THE DISH HAD A <br /> FISHY FLAVOR. COMPLAINANT HAS A SHELLFISH ALLERGY. THEY CALLED THE RESTAURANT AND SPOKE WITH <br /> AN EMPLOYEE AND THEY EMPLOYEE INFORMED HER THAT THEY USE OYSTER SAUCE IN THE MU SHU PORK. <br /> COMPLAINANT ALLEGES THE TAKE OUT MENU DID NOT MENTION THE OYSTER SAUCE AND THEY FEEL IT IS <br /> VERY DANGEROUS AND IRRESPONSIBLE OF THE RESTAURANT TO LEAVE THAT OUT. <br /> VIOLATIONS AND CORRECTIVE ACTIONS <br /> Items listed on this report as violations do not meet the requirements set forth in the California Health and Safety Code commencing with section 7; <br /> 113700.All violations must be corrected within specified timeframe. Violations that are classified as"MAJOR"pose an immediate threat to public health <br /> and have the potential to cause foodborne illness.All major violations must be corrected immediately.Non-compliance may warrant immediate closure of <br /> the food facility. <br /> OVERALL INSPECTION NOTES AND COMMENTS <br /> FOOD ITEM--LOCATION--TEMP°F--COMMENTS <br /> No Temperature Data Collected <br /> NOTES <br /> As per operator, he is aware of the complaint. Operator stated that the customer did her order over the phone and didn't <br /> mention any sort of food allergy.
