See attached payment ledger dated <br /> 09/26/ 17 in the amount of $ 3 , 093 . 00 check # 17698 made out to SJCEHD . Also, to date we have not <br /> received our 2019 permit to operate from your office, it was paid on 09/27/18 in the amount of <br /> $ 3, 714 . 60 check # 19203 . ( see attached a/p listing for SJCEHD ) <br /> Please inform us when the 2018 permit was mailed and when the 2019 will be mailed . <br /> 201 : <br /> Corrected on site <br /> 209 : <br /> Prior to 12/6/18 the UST visual inspection were done monthly because we were not informed that the <br /> regulation had changed October 1 , 2018 to every 30 days . Since 12/6/ 18 all inspections have been done <br /> within the new 30- day period . <br /> In the future the DO will verify that all the tank and UDC sensors are printed on the alarm history report <br /> and the report will be properly reviewed and all alarms have been responded to and documented <br /> correctly . <br /> 306 : <br /> Pig mats will not be used in the future and all sensors will be able to detect a leak at the earliest <br /> opportunity . <br /> 313 : <br /> Elite IV has been scheduled to repair the red dyed diesel meter on pump 13/ 14 . <br /> 319 : <br /> Elite IV has been scheduled to replace the existing DFC spill bucket with one that will contain 5 gallons . <br /> Please note that the existing bucket did pass the leak test that Afforda -Test performed and will be in the <br /> report that is submitted to EHD . <br /> 320 : <br /> When Elite IV replaces the 5 -gallon spill bucket note above ( 319 ) they will do the new overfill prevention <br /> equipment test and do any repairs that are required . <br /> Stephen Kludt owner/operator/ DO <br />
