The permit has been approved <br /> with the following conditions : <br /> 1 . Contact the local fire department and the San Joaquin Valley Air Pollution Control District for <br /> any applicable permits and requirements . <br /> 2 . Any changes in information and scope of work must be submitted to EHD for approval prior to <br /> commencing work . All new addenda supersede the previous ones . <br /> 3 . Contact Shewit Woldearegay at (209) 468- 0216 to arrange the following inspection <br /> appointments ( a 48- hour notice is required for inspection appointments ) . Please be aware that <br /> inspections for emergency work must be scheduled within five days of permit approval . <br /> a . Spill bucket test : Verify minimum 5-gallon capacity and pass a one hour hydrostatic test ( or <br /> equivalent) . Proper drainage must also be verified . <br /> b . Overfill prevention verification : Measurement of drop tubes to verify 95 % tank volume shut <br /> off. Verification of flapper functionality . Ensure that all pertinent tank charts and <br /> calculations are available for review at the time of inspection . <br /> c . Verification that ball float (s ) , if installed , do not interfere with the installed flapper valve . <br /> d . Since concrete will be broken near the tanks , a secondary containment test must be <br /> performed on the tanks once concrete is poured in accordance with the manufacturer' s <br /> guidelines . <br /> Please be aware that the facility cannot receive fuel in the 87 , 91 , and diesel tank prior to <br /> the EHD inspection and approval . <br /> 4 . Any individual performing the work of a service technician must meet the following <br /> qualifications : <br /> a . Possess or be employed by a person who possesses one of several permissible valid , <br /> current contractor' s licenses issued by the Contractors State License Board ( may be issued <br /> to the installation company) , or a Tank Tester' s license issued to the individual by the State <br /> Water Board , and <br /> b . Possess or work under the direct and personal supervision of an individual physically <br /> present at the work site who possesses a valid , current California UST Service Technician <br /> certificate issued by the International Code Council ( ICC ) , and <br /> 1868 E .
