The site map is missing a loading area. No storm <br /> drains should be stated on the site map. <br /> A site map shall contain a north orientation, loading areas, internal roads, adjacent streets, storm and sewer drains, <br /> access and exit points, emergency shutoffs, evacuation staging areas, hazardous material handling and storage <br /> areas, and emergency response equipment. If a site map element is not applicable for your facility then list it on the <br /> map and label as"NA". Immediately log into the California Environmental Reporting System (CERS)at <br /> http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for approval. <br /> This is a minor violation. <br /> 11 HSC 25505(a)(4) Failed to provide or document initial and/or refresher training to appropriate personnel. <br /> The business has a narrative stating "Please note that Verizon historically used three separate digital training <br /> courses to satisfy the annual training requirements set forth by Health and Safety Code§25505. The content of <br /> these courses was consolidated into a single course,which, beginning in 2020,will appear on training records as <br /> "Spill Prevention and Response")",the only employee training record that were uploaded in California Environmental <br /> Reporting System (CERS)was for Hazardous Communication training. No employee training records were provided <br /> for the Spill Prevention and Response course. Please complete the Spill Response and Prevention training. <br /> The business plan shall include provisions for ensuring that appropriate personnel receive initial and annual <br /> refresher training.All employees shall be trained in safety procedures in the event of a release or threatened release <br /> of a hazardous material, including, but not limited to,familiarity with the following plans and procedures: <br /> (A) Immediate notification contacts to the appropriate local emergency response personnel and to the unified <br /> program agency. <br /> (B)Procedures for the mitigation of a release or threatened release to minimize any potential harm or damage to <br /> persons, property, or the environment. <br /> (C) Evacuation plans and procedures, including immediate notice,for the business site. <br /> This training shall be documented electronically or by hard copy and shall be made available for a minimum of three <br /> years. <br /> Immediately provide employee training for appropriate personnel and submit a copy of the training records to the <br /> EHD. <br /> This is a Class II violation. <br /> Overall Inspection Comments: <br /> An inspection report was provided to the facility operator on the day of inspection. The report was amended and <br /> replaces the initial report. Upon further review violation number#5 is not applicable and has been removed from <br /> the inspection report on 9/3/2020. The business 30 days of noncompliance will be on 10/3/2020. <br /> To minimize person to person contact EHD is choosing to write the name of person receiving the report instead <br /> of having them sign. Starting September 1, 2018, all in-office CERS help will be provided at EHD hourly rate <br /> ($152).
