SAN <br /> J D �� I Environmental Health Department <br /> COUNTY— <br /> Underground Storage Tank Program Inspection Report <br /> Facility Name: Facility Address: Date: <br /> KWIK SERVE 950 W ELEVENTH ST, TRACY August 01,2019 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 209 CCR 2716(a-e) Failure to conduct the designated UST operator visual inspection at least once every 30 days. <br /> The designated operator failed to check and note all required items on the visual inspection reports: <br /> -The"N/NA"in sections VII through XI were not fully explained on the inspection reports. <br /> -Section V of the inspection reports were missing follow up actions about scheduling an Overfill Prevention <br /> Equipment Test. <br /> -The designated operator answered the question regarding sump sensor locations but did not list which sumps were <br /> visually inspected. <br /> -The designated operator documented visual inspections of sumps, but left the question regarding sump sensor <br /> locations blank. <br /> During the visual inspection,the designated operator shall check all required testing and maintenance for the UST <br /> system have been completed and shall check all required items on the report. Ensure that designed operators <br /> performing visual inspections at this facility are including all of the required information on the reports. <br /> This is a minor violation. <br /> 313 HSC 25290.1,25290.2, 25291 Failure to construct, operate, and maintain primary containment as product-tight. <br /> Approximately 1 cup of liquid was observed in UDC 7/8, indicating a leak in the primary piping.According to the <br /> service technician on site, a leak might be coming from the diesel product meter.All primary containment for the <br /> UST system must be tight. Immediately have a properly licensed,trained, and certified contractor repair or replace <br /> the failed component under permit and inspection of the EHD. <br /> This is a minor violation. <br /> 320 CCR 2635, 2637.2,2665, 2712 Failure to comply with the installation, inspection, or record keeping of the overfill <br /> prevention. <br /> Overfill prevention testing was conducted on February 28, 2019. This testing was required to be conducted by <br /> October 13, 2018. Ensure all future testing is conducted within required time frames. <br /> This is a Class II violation. <br /> Overall Inspection Comments: <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> documenting the corrective actions that have been or will be taken for each violation, and any supporting <br /> paperwork, by 8/31/2019. <br /> Starting September 1, 2018, all in-office CERS help will be provided at EHD hourly rate ($152).
