S A N J 0 A Q I I I Irl Environmental Health Department <br /> CCUNTY <br /> Small Quantity Hazardous Waste Generator Inspection Report <br /> Facility Name: Facility Address: Date: <br /> DICKS SPORTING GOODS#1297 2770 REYNOLDS RANCH PKWY, LODI August 31, 2020 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 110 CCR 66262.40(a) Failed to keep signed copy of manifests from the designated facility for three years. <br /> OBSERVATION: According to a hazardous waste storage area weekly inspection checklist dated 6/9/20, <br /> completed by Yazmine Trevizo, and found in the hazardous waste storage area, a spray paint can and two carbon <br /> dioxide cylinders were disposed into the Hazardous Waste Aerosols container the week of 6/9/20. However, at the <br /> time of inspection, only one carbon dioxide cylinder was found in the hazardous waste storage area,which had been <br /> placed in the storage area on the date of inspection by Yazmin.Yazmin stated that Stericycle,the facility's <br /> hazardous waste hauler, had picked up spray paint can and the two carbone dioxide cylinders, but didn't leave the <br /> disposal records on-site. Robert Leal stated that the store usually has to request the disposal records from <br /> Stericycle on a date after the waste has already been picked up. <br /> REGULATION GUIDANCE: A hazardous waste generator shall keep a copy of each manifest signed in accordance <br /> with section 66262.23(a)for three years or until the generator receives a signed copy from the designated facility <br /> which received the waste. This signed copy shall be retained as a record for at least three years from the date the <br /> waste was accepted by the initial transporter. <br /> CORRECTIVE ACTION: Provide a copy of disposal records for the pick up of the waste spray paint can and two <br /> carbon dioxide cylinders after 6/9/20. Provide these records and a corrective action statement to the EHD within 30 <br /> days. <br /> This is a Class II violation. <br /> Overall Inspection Comments: <br /> Notes: <br /> -The facility generated .0045 tons of hazardous waste in 2019, according to the facility's disposal records <br /> reviewed on-site. <br /> -OBSERVATION: A hazardous waste container was observed on-site with incomplete labeling. The container <br /> was: <br /> 1. A Hazardous Waste Aerosols storage container storing a 12 oz carbon dioxide cylinder was missing the <br /> generator name and the accumulation start date on the hazardous waste container label. <br /> REGULATION GUIDANCE: All hazardous waste containers shall be marked with the following information: the <br /> words "Hazardous Waste", name and address of generator, hazardous properties, physical state, composition <br /> (contents), accumulation start date <br /> CORRECTIVE ACTION: Yazmine Trevizo, freight flow lead, corrected the hazardous waste label on the date of <br /> inspection. No further further action is required. <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> documenting the corrective actions that have been or will be taken for each violation, and any supporting <br /> paperwork, by 9/30/2020. <br /> To minimize person to person contact EHD is choosing to write the name of person receiving the report instead <br /> of having them sign.
