Immediately log into the California Environmental Reporting System (CERS) at http:cers.calepa.ca.gov/and upload the required documents. This is a minor violation. 104 CCR 2711 UST operating permit application for facility and tank information not submitted or current. The UST Tank Information on UDC are not current in CERS. The UDC leak detector name of manufacturer and model are missing. Any change of information must be updated in CERS within 30 days of the changes. Immediately log into CERS, update the required information, and submit for review by the EHD. This is a Class II violation. 105 CCR 2632(d)(1), 2634(d) Failed to have an approved UST monitoring plan. The monitoring plan is not current and/or not approved by the EHD. The moitoring plans list the monitoring panel for the tanks , piping and UDC as Veeder root TLS 350. The panel on site is Gilbarco EMC. The monitoring plan must be uploaded to the California Environmental Reporting System (CERS). Immediately log into CERS, make the necessary changes, and submit for review by the EHD. This is a minor violation. 202 CCR 2715(c)(3), 2715(c)(4) Failure to maintain the "Facility Employee Training Certificate" on-site (after October 1, 2018). The Facility Employee Training Certificate dated 05/06/19 contained no CERS ID.The Facility Employee Training Certificate shall contain accurate information for all employees trained. Update the Facility Employee Training Certificate to contain the accurate information and submit a copy of the updated form to the EHD. This is a minor violation.
