k' ■ K L E I N F E L D E R <br /> Task 4—Reporting <br /> Upon completion of the field activities, a short letter report documenting our observations will be <br /> prepared for client and agency review. The report will include: <br /> • A description of the field activities, observations, and protocols <br /> • A well destruction log will be submitted for each well/piezometer to the Department of <br /> Water Resources (DWR) <br /> All information gathered during the study by Kleinfelder is considered confidential and will be <br /> released only upon written authorization from the client or as required by law. California law <br /> requires a person to inform the state if a situation is encountered that can be considered an <br /> immediate endangerment to the public's health or welfare and/or to the environment. Therefore, <br /> the client will be contractually bound to make said disclosures in lieu of Kleinfelder, its owners, <br /> principals, employees and subcontractors. <br /> CLIENT RESPONSIBILITIES AND ASSUMPTIONS <br /> This proposal and the attached cost estimate are based on the following assumptions: <br /> • Adequate work space and access will be provided. If the drilling equipment cannot be <br /> positioned, alternative methods should be investigated. <br /> • The piezometers are to be pressure grouted with a truck mounted drill rig. The <br /> domestic/irrigation well will be tremmie grouted using cement sand slurry. If the <br /> estimated volume of grout/bentonite slurry used varies significantly from the actual <br /> amount used, costs will be adjusted accordingly on a time and expense basis. <br /> • The client will provide access to and from the wells/piezometers to be destroyed. <br /> Kleinfelder is not responsible for additional costs incurred from inclimate weather <br /> conditions. <br /> • This proposal is based on the presumption that the client will clearly mark utilities and <br /> will provide any and all available survey maps or other data to determine the location of <br /> tanks, existing structures, underground utilities and services. <br /> • The client should be aware that penetrating the site's surface is inherently risky. It is <br /> impossible to determine with certainty the precise location of all structures which may be <br /> buried in the ground. Kleinfelder's fee is not adequate to compensate for both the <br /> performance of the services and the assumption of risk of damage to such structures. <br /> • Underground Services Alert (USA) at 1-800-642-2444 provides a partial location service <br /> free of charge for major utility lines.
