Current fnancial responsibility documents have not been submitted to the EHD. -The Certification of Financial Responsibility marks $2 million annual aggregate in Section A, but only$1 million in section C. -The Letter from the Chief Financial Officer includes asset and liability amounts that have not been updated in over five years. -The dates on both documents were whited out and rewritten, without obtaining new signatures. Current financial responsibility documents are required to be submitted annually. Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, and upload the required documents. This is a minor violation. 106 CCR 2632(d)(2), 2634(e) Failed to have an approved UST response plan. Section V of the response plan was left blank. The response plan must be uploaded to the California Environmental Reporting System (CERS). Immediately log into CERS, upload the completed form, and submit for review by the EHD. Ensure a copy is available for inspection at any time. This is a minor violation. 107 CCR 2632(d)(1)(C) Plot plan/site map not submitted or failed to completely show where monitoring is performed. An accurate UST Monitoring Site Plan was not submitted. The map submitted in CERS shows an inaccurate sump layout for the 91 and diesel tanks, does not include annular sensors, and lists an incorrect street name east of the facility. A site plan must be submitted identifying the locations where monitoring will be performed. Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/and upload a copy of the UST Monitoring Site Plan. This is a minor violation. 113 CCR 2638(c), 2638(d) Failure to submit"Monitoring System Certification Form"within 30 days of completion of the test. Annual monitoring system certification, leak detector testing, and spill container testing was performed on 4/23/18 and a test report was submitted on 7/11/18, two months late. A copy of the test report must be submitted within 30 days of the tests. Take all necessary precautions to ensure testing and submittal of test reports are performed in a timely manner.
