This information has not been received <br /> resulting in a non-compliant status for this facility. <br /> Failure to correct the violations will result in continuing/additional violations. <br /> Note: All EHD staff time associated with failing to comply,including the issuance of this letter,is <br /> billed at the current hourly rate($122).
